Telehealth Informed Consent

This consent explains the use of telehealth for medical services provided by DrTelx and its affiliated independent professional entities and licensed healthcare providers. Pretty Girls Peptide coordinates non-clinical technology and administrative services and does not practice medicine.
What telehealth includes
Telehealth may include online health questionnaires, secure messaging, telephone calls, video visits, electronic prescribing, remote review of records or photographs, and other electronic communications. Your provider may require additional information, identity verification, laboratory testing, a live consultation, or an in-person evaluation before making a clinical decision.
Potential benefits
Telehealth can provide convenient access to licensed providers, reduce travel, support timely communication, and facilitate ongoing monitoring and prescription coordination. These benefits are not guaranteed and depend on your location, condition, technology, and provider judgment.
Potential risks and limitations
Telehealth may not permit the same physical examination, testing, observation, or immediate intervention available in person. Information may be incomplete or misunderstood. Technology may fail, communications may be delayed, unauthorized access may occur despite safeguards, and a provider may determine that telehealth is inappropriate. Treatment may need to be delayed or discontinued until you obtain additional or in-person care.
Provider relationship and clinical independence
A questionnaire or eligibility screen alone does not create a provider-patient relationship. That relationship is established only when an authorized provider accepts you for care. The provider independently reviews your information and decides whether to prescribe, request more information, recommend another treatment, require in-person care, or decline treatment. Pretty Girls Peptide does not make these decisions.
Prescriptions
A prescription is issued only when a provider determines it is medically appropriate and legally permitted. No prescription or particular medication is guaranteed. The provider may change, pause, or discontinue treatment based on response, side effects, monitoring, availability, safety, or new information.
Compounded medications
Some prescribed medications may be compounded. Compounded medications are not FDA approved, and FDA does not review them for safety, effectiveness, or quality before marketing. Compounded products may differ in formulation, appearance, packaging, concentration, inactive ingredients, storage, and instructions. Discuss the risks, benefits, and alternatives with your provider.
Pharmacy fulfillment
If prescribed, the order may be sent to an independent licensed pharmacy, including EPIQ Scripts, Foothills Professional Pharmacy, Emerald Compounding Pharmacy, or another pharmacy in the network. The pharmacy is responsible for compounding or dispensing, labeling, counseling, and shipping. Availability and delivery times vary and are not guaranteed.
State-specific consultation requirements
You must be physically located in a jurisdiction where your provider is authorized to practice at the time of care. Asynchronous questionnaire-based care may be available in AK, AL, AR, AZ, CA, CO, CT, DE, FL, GA, HI, IA, ID, IL, IN, KY, MA, MD, ME, MI, MN, MO, MT, NC, ND, NE, NH, NJ, NV, NY, OH, OK, OR, PA, SC, SD, TN, TX, UT, VA, VT, WA, WI, and WY. A live consultation is required before treatment in DC, KS, LA, MS, NM, RI, and WV. A provider may require a live or in-person visit in any jurisdiction when clinically necessary or legally required.
Privacy
Telehealth information is handled under applicable privacy and security laws. DrTelx, affiliated provider entities, and pharmacies may provide separate privacy notices governing medical and pharmacy records. Electronic communications and household devices can create privacy risks; use a private location and secure device when possible.
Your responsibilities
You agree to provide accurate and complete information about your identity, physical location, medical history, medications, allergies, pregnancy status, symptoms, and changes in health. Follow treatment, monitoring, storage, and follow-up instructions; report side effects and medication errors promptly; do not share medication; and seek in-person or emergency care when advised.
Alternatives
Alternatives may include an in-person visit, treatment by another provider, use of an FDA-approved medication when available and appropriate, non-medication care, or declining treatment. You may ask the provider about alternatives before consenting.
Emergencies
Telehealth services offered through this platform are not emergency services. Call 911 or go to the nearest emergency department for a medical emergency. Call or text 988 for a mental-health crisis or suicidal thoughts.
Voluntary consent and withdrawal
Your use of telehealth is voluntary. You may ask questions and withdraw consent to future telehealth services by notifying the provider, subject to care already provided, prescription or pharmacy activity already initiated, and legal recordkeeping obligations. Withdrawal may require transition to in-person care or discontinuation of services.
Acknowledgment
By accepting this consent electronically, you confirm that you are at least 18, have read and understood this information, have had the opportunity to ask questions, consent to telehealth, and authorize relevant information to be shared among Pretty Girls Peptide, DrTelx and affiliated providers, pharmacies, and service providers as permitted by law to coordinate requested care.
